| TherapyAppointment | 15¢ per electronic claim. ERA posting is free. Claims connect through your Office Ally account after a discount form. | 7¢ per benefits lookup. | Not listed. Ask on the demo. |
| Sessions Health | $0.25 down to $0.19 per claim by monthly volume. ERA auto-post once enrolled. No setup, annual, or monthly minimum on the pricing FAQ. | $0.15 per real-time report. | Automatic superbills through the client portal. |
| ICANotes | Electronic and paper claims. Remittance auto-post. Claim fee is quote-only. | Eligibility checks and authorization tracking. | Superbills, statements, and CMS forms in the product. |
The next table keeps the same five tools and moves to the money that does not sit on the claim fee.
| Software | Client payments | Denials | EHR link |
|---|---|---|---|
| SimplePractice | Invoicing is in Essentials. Card rate not published. Ask. | Navigator Claim Assistant for rejected or denied claims. Essentials has pre-submit checks. | The EHR. Claims file in the chart. |
| TherapyNotes | 3.1% plus 30¢. Cards charged in the product. | Rejection alerts on the To-Do list. Resubmit from status tracking. | The EHR. The claim pulls from the record. |
| TherapyAppointment | Finix, 2.9% plus 30¢. Cards on file. Copay tied to the visit. | Problem-claim flag on the billing dashboard. | The EMR. No separate billing product required. |
| Sessions Health | Stripe, 2.9% plus 30¢. Autopay. No added platform fee. | Flags denials and incomplete claims for correction. | The EHR. Batch and secondary claims from the chart. |
| ICANotes | Self-pay statements and payment tracking. Card rate not published. Ask. | Eligibility and auth tracking aimed at fewer denials. Denial follow-up is a separate RCM service. | One record for the note and the bill. Export to another biller is offered. |
Price and the contract sit on their own rows so a wide screen does not hide them.
| Software | BAA | Price |
|---|---|---|
| SimplePractice | BAA entered when the account is created. Read the applicability clause before you rely on it. | Starter $49, Essential $79, Plus $99, then $74 per added clinician. Groups of 6 or more are custom. Claims are extra. |
| TherapyNotes | Not shown on the public security page. Ask before you store PHI. | Solo $69. Group $79 plus $50 per added clinician. Non-clinical staff free on group plans. |
| TherapyAppointment | Their vendors must sign a BAA. The agreement they offer you is not published. Ask. | After the trial: $10, then $39, then $59 by booked sessions. A higher tier does not drop on its own. |
| Sessions Health | HIPAA and HITECH are claimed. A BAA is not published. Ask. | Free up to 3 clients. Then $39, plus $29 per added practitioner. Claims and eligibility are extra. |
| ICANotes | HIPAA and HITECH are claimed. A BAA is not published. Ask. | Non-prescribing $75. Prescribing $213 plus a $99 activation fee. Notes-only $55 has no billing tools. Claims are quoted. |

You are comparing the best billing software for therapists when rent depends on insurance, superbills, or both. The five rows are a shortlist, not a winner. If you are still looking for the best billing software for therapists after the subscription price looks fine, add the claim fee, the eligibility fee, and the card rate before you book a demo. A tool that cannot show the row you bill every week is not a fit, even if the calendar is pretty.
If a vendor will create, receive, maintain, or transmit protected health information on your behalf, 45 CFR § 164.502(e) allows that only when you obtain satisfactory assurance, documented through a written contract or other written agreement, that the vendor will safeguard the information. A security page that describes encryption does not replace that contract. Read it, or have counsel read it, before a client name goes in.
What the subscription line leaves out
Write a normal insurance week before you compare logos. Count claims, eligibility checks, and card charges. Then add those fees to the monthly plan.
SimplePractice Essentials is included on every plan. The claim is still $0.39, and a manual mental health coverage check is $0.15. Navigator, at $1.79 per claim, is the add-on with specialists, automatic benefits estimates, and Claim Assistant. Do not price the $49 Starter plan as if claims were free. The insurance FAQ says the old free-claim allotment on Essential and Plus is gone. Every submitted claim bills at your rate.
TherapyNotes prints the usage fees on the pricing page: 14¢ for the electronic claim, 14¢ for the ERA claim, and 14¢ for the eligibility request. A visit that runs all three is 42¢ before the card fee. Non-clinical staff are free on the group plan, which matters if a biller logs in and does not see clients.
TherapyAppointment’s ERA posting is free. The claim is 15¢, and the billing page says you connect your own Office Ally account at that rate after a discount form is approved. Eligibility lookups are 7¢. The session tiers ($10, then $39, then $59) do not step back down if a quiet month follows a full one. Ask support before you assume a vacation month returns you to $10.
Sessions Health prices claims on a volume ladder, from $0.25 down to $0.19, and includes ERA posting once you enroll. Eligibility reports are $0.15. Stripe’s 2.9% plus 30¢ is the card rate, with no extra platform fee on top. Administrators are unlimited on the Professional plan. The free plan stops at three active clients.
ICANotes does not print a claim fee. The pricing page says claims vary and tells you to contact sales. The non-prescribing plan at $75 is the therapy billing plan. The prescribing plan at $213, plus a $99 activation fee, is the one that adds e-prescribing and labs. Notes-only, at $55, does not include billing tools. If you prescribe, do not budget the notes-only rate and discover the claim path later.
Put the subscription, the per-claim fee, the eligibility fee, and the card rate on one sheet before the demo. The cost model in the Therapy Software Buying Pack is that sheet. It is not a claims scorecard.

Questions to take into the demo
Print this when you are choosing the best billing software for therapists, and fill it while you click. A pass is a screen. A promise to turn the feature on after go-live is a blank.
A filled card looks like this. The numbers are the published rates. The demo still has to show the screens.
Take the same card to the other four. Change only the name and the fees. If you need a pass-fail walkthrough for one synthetic 90837, use the EHR billing demo scorecard. This comparison tells you which product to open. That scorecard tells you which click is a fail.
Psychiatrist billing software
If you are a psychiatrist, you still score those nine fields. The extra question is whether the plan you are quoted can carry an evaluation-and-management visit and, if you prescribe, the prescribing tools. The best billing software for therapists does not become a different product because you also write 99214s. You run the same test, and you add that question.
ICANotes is the row built for that work. Their practice-management page describes E&M coding support on 99213 to 99215 claims, and the prescribing plan is the one priced at $213 a month. A counselor who does not prescribe should not buy that plan to get claims. A psychiatrist should not stop at a notes-only rate and assume billing is included.
TherapyNotes, SimplePractice, Sessions Health, and TherapyAppointment can still be the right claim path for a therapy-heavy psychiatry practice. Confirm secondary claims, incident-to billing, and where a denied E&M visit shows up. Sessions Health states incident-to billing and secondary claims on its billing page. TherapyNotes states primary and secondary electronic claims on its billing page. If those sentences are missing for the tool you prefer, the demo has to produce the screen.
If you searched for the best medical billing software for mental health, stay on these fields. A physician billing suite that has never opened a 90837 is a different product. So is a data export that dumps payments into a spreadsheet. The test is whether the denial and the remittance land on the same chart you will defend at audit.
Where the chart decision lives
This comparison is the claim path. It is not the whole practice system.
If you are choosing notes, scheduling, telehealth, and the chart together, start with the best EHR for private practice shortlist, then come back here for the billing rows those products do not settle. If you need the week-to-week billing routine, eligibility before the first session, who owns a denial, and how a client balance gets collected, use the insurance billing workflow for therapists.
Emosapien does not belong in the table. It drafts notes for your review. It does not file a claim, post an ERA, or hold the chart of record. If you want that notes layer beside the billing system you keep, the feature set is the place to check what it does today, and you can create a free account without treating it as billing software.

Print the cost model from the pack before you book the second demo. The best billing software for therapists is the one whose failed row you can point at, not the one with the lowest monthly number. Write the plan, the claim fee, the eligibility fee, and the card rate in the same units. A tool that wins on the monthly price and loses on 200 claims is not the cheaper tool.
