Vendor facts are based on each vendor’s own public US pages, read October 10, 2026. Prices and terms can change after that date. Confirm the live account before you buy.
A PHQ-9 and a GAD-7 are useful in the chart when they go out on a cadence you already use, score themselves, and sit next to the note you are about to write. You still read the score. The software does not. The best EHR for digital intake and assessment automation is the row that finishes that loop in one product.
The intake workflow to test in the demo
Do not use a real client. Read this case as written. Change only the payer name to one you actually bill.
Copy the blank sheet into the next demo. If a line stays empty, that line is still open.
Billing that has to keep up with the packet
A comparison of EHR billing software with deep integration features is the same sitting as the intake test. The signed packet and the PHQ-9 graph do not pay rent if the claim lives in a second product.
SimplePractice prints Insurance Essentials on every plan and treats Insurance Navigator as extra. TherapyNotes files electronic claims at 14 cents per electronic claim. Jane puts insurance on an add-on. Upheal’s EHR plan prints $0.30 a claim and $0.15 an eligibility check. Blueprint’s pricing page lists $0.30 a claim and $0.15 an eligibility check, marked free until September 2026, so ask which applies to you now. Watch one synthetic 90837 leave. A brochure line is not a remittance.
If the claim path is the whole buy, score it on the EHR billing and insurance claims demo. Deep billing integration is one column on the best EHR for digital intake and assessment automation, not a second purchase.
What to confirm on each row
SimplePractice
Intake forms and e-signature. Support documents name intake forms, consent forms, and scored measures shared through the Client Portal, with a countersign step on completed files.
Scheduled measures. Recurring frequencies include after every appointment, before every appointment, before every other appointment, every 2 weeks, and every 4 weeks.
Scoring and graphs. Completed measures auto-score. Progress sits on the client’s Measures tab.
Open question. Which plan you will buy still has to match reminders, extra clinicians, and Navigator. Confirm that on the live pricing page.
TherapyNotes
Intake forms and e-signature. TherapyPortal is in the subscription. Clients can review, complete, and electronically sign. Custom forms can take a drawn or typed signature.
Scheduled measures. Outcome measures can go in the intake packet or as a later portal request, and some measures can stay available on the portal. A named every-two-weeks send is not on the outcome-measures page. Set the cadence in the demo or write “not offered.”
Scoring and graphs. PHQ-9, GAD-7, and a wider library auto-score. Results are available when the client submits.
Open question. The public pricing page prints 14 cents per electronic claim. Confirm ERA and eligibility in the demo.
Jane
Intake forms and e-signature. Intake can send when the client books. Consents can require a drawn or typed signature.
Scheduled measures. Clinical surveys can go out before or after a booked visit, or at online booking. Scored questions become an outcome-measure survey.
Scoring and graphs. Jane tallies scored survey questions. Ask the demo to show the trend across visits, not only today’s total.
Open question. US insurance billing is an add-on. Add $20 plus $5 per full-time license to the plan fee before you call Jane cheap.
Upheal
Intake forms and e-signature. You send intake forms and consent from the Forms page or from the client’s profile.
Scheduled measures. You can send a PHQ-9 or GAD-7 from the chart. Confirm whether a two-week cadence is automatic or a manual resend.
Scoring and graphs. The Insights tab plots submissions, including change since last time and since the first score.
Open question. The EHR plan prints $0.30 a claim and $0.15 an eligibility check. AI Notes is $39 a month. EHR is $1 a session. EHR Pro is $99 a month. Submit one test claim before you treat any one of those numbers as the whole bill.
Blueprint
Intake forms and e-signature. The all-in-one EHR path lists customizable intake and consent forms with digital signatures.
Scheduled measures. You can assign assessments and worksheets. Suggested assessments sit in the AI layer. Confirm the automatic cadence in the demo.
Scoring and graphs. Built-in outcome measures and progress tracking are on the pricing story. Open a PHQ-9 trend, not a library screenshot.
Open question. Alongside your EHR is $0.99 a session, or $69 a seat with unlimited sessions. All-in-one is $1.49 a session. The pricing page lists $0.30 a claim and $0.15 an eligibility check, marked free until September 2026. Card processing is described as waived until February 2027, then 3.15 percent plus $0.30. Write those lines separately.
Related work you will still need
The therapy intake checklist is the internal control: consent, screening, risk review, and logistics before session one. The intake form generator is a starting packet if you do not already have one. Neither one schedules a PHQ-9.

Keep the chart. Run the cadence beside it
Emosapien is not a sixth EHR on this shortlist. It does not hold the signed consent as the chart of record. It does not file the 90837. It can send PHQ-9, GAD-7, ORS, and related measures on the cadence you set, score them, and put the change next to the treatment goal. You review every draft note.
