GLP 1 for Binge Eating Disorder: What You Track
Outline
Scroll the table sideways to view every column
| What to track | Why it matters | What to ask | When to contact the prescriber |
|---|---|---|---|
| Restriction and skipped meals | A smaller plate can be the drug or a diet rule | Fear foods still out, or just not hungry? | Persistent skipping, dizziness, or a week that looks like a fast |
| Binge urges and loss of control | Lower appetite is not the same as recovery | Any episode with loss of control this week? | Urges jump after a missed dose or a pause |
| Body image and checking | Rapid change can spike shape overvaluation | Weighing, mirror checking, clothes as the measure? | New purging or compulsive exercise |
| Mood, sleep, irritability | A mood shift is data for the prescriber, not a side note | Sleep, hopelessness, interest in the week? | Acute hopelessness or self-harm talk |
Dana texts at 7:12 p.m. Her internist started a weekly GLP-1 last month for weight. Dinner is a few bites. Saturday still has a binge. She wants you to say the shot is working.
You are not the prescriber. GLP 1 for binge eating disorder is not on the Wegovy label. Your job is the hour: what changed in restriction, binge urges, body image, and mood, and what that internist needs in writing.
Educational content for licensed therapists, not medical, legal, or prescribing advice. Medication choice, dose, and titration belong to the prescriber. Do not start, stop, or change a GLP-1.
What the labels actually allow
Wegovy (semaglutide) is a GLP-1 receptor agonist. The DailyMed label lists it, with a reduced-calorie diet and more activity, for long-term weight reduction, for lowering major cardiovascular events in adults with established cardiovascular disease and obesity or overweight, and for noncirrhotic MASH with moderate to advanced fibrosis. Binge eating disorder is not on that list.
If someone asks you about GLP-1 for binge eating as a treatment, the honest sentence is: not on this label, early evidence, prescriber’s call.
Lisdexamfetamine (Vyvanse) is FDA-labeled for moderate to severe binge eating disorder in adults. That label also says it is not indicated for weight loss. If the live question is that drug, use Vyvanse for binge eating. Stay here when the shot already in the picture is a GLP-1.
| Product | What DailyMed labels | Binge eating disorder |
|---|---|---|
| Wegovy (semaglutide) | Weight reduction, CV risk reduction, MASH | Not labeled |
| Vyvanse (lisdexamfetamine) | ADHD; moderate to severe BED in adults | Labeled in adults |
You do not need to recite every indication. You need to stop a session from treating a weight-loss shot as if it were approved for the eating disorder.
What the evidence shows so far
A 17-week pilot randomized trial of liraglutide in 27 adults with binge eating disorder and BMI at or above 27 found binge episodes fell in both the drug and placebo groups. The difference was not statistically significant. Weight loss was larger on liraglutide. The authors also reported a pharmacy dispensing error. That is a signal to study further, not a reason to treat GLP 1 for binge eating disorder as settled.
A retrospective clinic cohort reported larger drops in Binge Eating Scale scores on semaglutide than on lisdexamfetamine or topiramate in that sample. It was open-label and not a randomized trial. A 2024 systematic review of GLP-1 receptor agonists for binge eating in binge eating disorder and bulimia nervosa called the early signals promising and said large blinded trials are still missing.
A 2026 rapid review of GLP-1 drugs used for obesity or type 2 diabetes found binge scores and food cravings often improved or stayed the same, and said limited data are available on eating behaviors and eating disorders. GLP-1 and eating disorders are not a finished evidence story. You still need a formulation.
So you do not sell the shot, and you do not scold a client who already started one. Keep the hour on regular eating, loss of control, and shape overvaluation. Eating disorder therapy techniques is the technique page for CBT-E, DBT-informed skills, and family-informed support. When a GLP-1 is already in the chart, keep the hour on what changed and what the prescriber needs in writing.

Restriction can hide inside a “good response”
The label warns about gastrointestinal adverse reactions, including severe ones, and about acute pancreatitis. Delayed gastric emptying is part of how these drugs work. In an eating-disorder history, that overlap is the clinical problem.
A client who “just isn’t hungry” may be having the expected appetite effect. They may also be using the shot as permission to skip, to delay, or to keep fear foods out. You cannot tell from portion size alone.
Ask what is still allowed on the plate. Ask whether they are proud of how little they ate. Ask whether dizziness, constipation, or vomiting is being treated as proof the drug is strong. Write the answers in the note as observations, not as a nutrition plan.
The same label warns of acute kidney injury due to volume depletion after gastrointestinal losses such as vomiting or diarrhea. If the week looks like a fast, they cannot keep fluids, or abdominal pain is severe and persistent, write the prescriber instead of waiting for the next medication visit to see if it settles.
Binge urges are not the same as appetite
GLP-1 for binge eating is often sold, in hallway talk, as a way to quiet “food noise.” Loss of control is not only noise. A client can eat less at lunch and still binge on Saturday after a fight, a scale number, or a skipped breakfast.
Track objective binge episodes or subjective loss of control the way you already would: count, context, and what happened after. If urges fall because meals are regular and skills are in use, that belongs to the therapy. If urges fall only while the shot is on board and jump the week a dose is missed, that belongs in the prescriber note.
Do not treat a quiet week as remission until you have asked about secret episodes, night eating, and compensatory behavior.
Body image and mood still need a sentence in the note
Rapid weight change can tighten shape overvaluation even when the number on the scale is the one the internist wanted. Checking, weighing, and clothes-as-progress show up in ordinary outpatient hours. Name them. If purging or driven exercise appears after the shot starts, write the prescriber and reopen eating disorder assessment rather than assume the medication is “working.”
Mood, sleep, and irritability are not optional extras. You are not diagnosing a psychiatric side effect from one session. You are writing what changed so the person who can change the prescription has the picture.
If hopelessness or self-harm talk is new, follow your risk protocol first, then tell the prescriber.
What you pass on, and what you do not
You observe, ask, document, and send. You do not pick a product, a dose, or a titration schedule. You do not tell the client to stop the pen because you dislike weight-loss culture, and you do not tell them to stay on it because a podcast was enthusiastic.
A usable note to the prescriber is short: what you tracked, over how many days, what the client reported, and the specific concern (persistent vomiting, skipped meals, new purging, mood drop). Put it on the same team document you already use. The eating disorder treatment plan template is the place for named medical and dietitian colleagues, not this page.
If there is no named prescriber, that is the first gap. A generalist outpatient therapist should not be the only adult watching a client with binge eating who is also on a GLP-1.
Keep the hour on the eating disorder
GLP 1 for binge eating disorder will keep showing up in search, in primary-care portals, and in the last five minutes of the session. Answer the search plainly: not on the Wegovy label, early data, you coordinate.
Then go back to regular eating, the binge sequence, and shape overvaluation. That is still the work. The tracker at the top of this page is how you keep the shot from swallowing the formulation.
Emosapien keeps the trackers and prescriber note with the treatment plan between sessions. The product does not prescribe, and it does not replace the internist.
