What to track Why it matters What to ask When to contact the prescriber
Akathisia (inner restlessness) Adjunctive aripiprazole listed akathisia far more often than placebo in MDD trials Can you sit through this hour, or is there an inner need to move? Same day if they call it unbearable or they stopped the tablet
Sedation and next-day fog Quetiapine XR somnolence was common in MDD adjunct trials and the usual reason people dropped out Are you late, napping, or missing morning work because of sleepiness? Same week if function holds; same day if they stopped driving or stopped coming
Weight and appetite Metabolic change is a class warning; clothes and appetite show up here before a lab does Have clothes tightened, and did any planned meal get skipped? Same week, named in the note, with their words
Mood load (one named scale) You need a number you can compare next week What was this week's total, and who scored it? Same week if the total jumps or they stop leaving the house
Suicidal thinking Adjunct labels carry the antidepressant boxed warning: watch for worsening, especially early or after a change Any wish not to wake up, or a plan you have not heard before? Same hour: stay, complete risk work, then notify
Missed tablets or extra tablets Adherence is an observation, not a lecture Did you skip, hide, or take an extra tablet this week? Same day if they stopped or doubled them
Compulsive urges Aripiprazole and brexpiprazole labels tell the prescriber to ask about gambling, shopping, sex, and binge eating Any new urge you cannot stop, including gambling or spending? Same day, in their words, no milligram from you

Authored by Andrew Evans, a clinical psychologist who writes about clinical operations, privacy, and getting the next clinician a note they can act on.

They sit down and say the psychiatrist added Abilify last Thursday because the antidepressant “wasn’t enough.” They want you to say whether they should skip tonight’s tablet so they can sleep.

You do not answer that question. Antipsychotics for depression are a prescriber’s product. Your hour is the tracking hour: restlessness, sedation, weight, mood, and risk. Write those lines. Send them.

Same day means you send the note before you close the chart. Same week means the prescriber can see it at the next booked slot, as long as you still wrote the observation in your own record today. If they ask what milligrams they should be on, say you do not set the dose. You will send the restlessness, sleep, and risk lines so the person who does set it has something true to read.

Educational content for licensed therapists, not clinical or prescribing advice. Product choice and dose belong to the prescriber. You observe, ask, document, and pass a short note.

Which bottles carry an FDA adjunctive indication

Antipsychotic drugs for depression are not one class protocol. Five adult adjunctive indications sit on current FDA labels. Immediate-release quetiapine is not the adjunctive MDD product. If they name a bottle that is not in this table, you still track the same lines. You do not recode the indication.

Scroll the table sideways to view every column

Product they may name FDA adjunctive role What you more often hear What you still send
Aripiprazole (Abilify) Adjunct to an antidepressant for adult MDD Inner restlessness, insomnia, new urges they cannot stop Akathisia, sleep, risk, adherence
Quetiapine XR (Seroquel XR) Adjunct to an antidepressant for adult MDD Heavy sleepiness, next-day fog, weight Sedation, meals, function, risk
Brexpiprazole (Rexulti) Adjunct to an antidepressant for adult MDD Restlessness, weight, compulsive urges Akathisia, gambling or spending, risk
Cariprazine (Vraylar) Adjunct to an antidepressant for adult MDD Restlessness that can show up weeks after a change Keep watching several weeks; send dated lines
Lumateperone (Caplyta) Adjunct to an antidepressant for adult MDD Dizziness, dry mouth, sleepiness, nausea Sedation, GI comments, mood total, risk

If they say “Seroquel” without XR, ask what is printed on the bottle and write that string. Immediate-release quetiapine is a different labeled product. You still track sedation and weight. You do not tell them they are on the wrong one.

The cariprazine label also tells the prescriber that adverse reactions can show up several weeks after a start or a change because of the long half-life. That sentence is not a titration rule. It is why you do not stop asking about restlessness just because week one was quiet.

If the bottle was started for schizophrenia, write that indication. For a recent switch onto a muscarinic product, use the new schizophrenia drug note, then still send today’s observation.

What the evidence gives you in the room

You are not picking an antipsychotic. You are sitting with one client who already has a bottle in the kitchen. A few facts change what you ask.

Fact Where it lives What you do with it
Five adult adjunctive MDD indications sit on current FDA labels Each product label, indications Write the name on the bottle they actually take
Adjunctive aripiprazole listed akathisia-related events in 25 percent versus 4 percent on adjunctive placebo in short MDD trials Aripiprazole label, adverse reactions Ask inner restlessness every visit. Do not call it motivation
In MDD adjunct trials, quetiapine XR somnolence was reported in 40 percent versus 9 percent on placebo, and it was the reaction that led people to stop Seroquel XR label Ask next-day fog before you assume they are non-adherent
Aripiprazole and brexpiprazole labels warn on intense gambling, shopping, sexual, and binge-eating urges the person may not flag as odd Each label, compulsive behaviors Ask once, privately. Put their words in the note
Antidepressants increased suicidal thoughts and behaviors in people 24 and younger; adjunct labels tell you to watch for worsening, especially early or after a change Boxed warning Ask risk every visit. A quiet total does not cancel a new wish not to wake up
Elderly patients with dementia-related psychosis treated with antipsychotic drugs are at increased risk of death; these products are not approved for that use Boxed warning, class If a family is treating confusion as depression, send that frame. Do not recode the chart
Treating a depressive episode can uncover mixed or manic symptoms Warnings, class Ask about less sleep with extra energy before you call it finally motivated

Aripiprazole as an adjunct beat placebo on depression totals in a six-week multicenter trial. That trial does not give you a milligram. It gives you permission to keep measuring antipsychotics for depression the same way you measure any other medication week: one instrument, then the table.

Score one instrument, then finish the medication lines

Use the scale you already named and can score today. Do not average two totals. Then finish the table. A PHQ-9 without restlessness, sedation, weight, risk, and adherence is a fragment. The next clinician cannot act on a fragment.

If the hour turns into a treatment-plan rewrite, pull goals from the problem you assessed, not from the brand name. The depression treatment plan template is the planning skeleton. Keep this visit’s medication lines short enough to paste.

If you need a chart shape for the psychiatry loop, the psychiatric progress note template is the note skeleton. Do not park a medication story there with no table lines beside it.

A note you can send the same afternoon

Weak: “Client started Abilify. Will monitor.”

Stronger, and short enough for a real chart:

Copy the blank fields when you want paper in the room.

What you never write

Do not write a suggested milligram. Do not write “consider increasing.” Do not guess the dose because they left the bottle at home. Do not interpret a lab you have not seen. Do not tell them to skip tonight so they can sleep.

A useful pass-on is dated, names the product they report, gives one mood total, quotes restlessness or sedation in their words, flags missed tablets, and states one question for the prescriber.

Ask about new stiffness, tremor, or tongue and mouth movements you have not seen before. Put those in the note. You are not scoring a tardive-dyskinesia scale in this hour unless that is already your job. You are handing the prescriber a dated observation.

Who gets the note

When consent allows, send psychiatry the total, the safety-item status, the restlessness line, the sedation line, and the adherence line. If a mandated-reporting duty applies, follow that statute rather than waiting on a courtesy loop.

If suicide is in the room, stay, complete the suicide risk assessment, then notify. A medication coordination card is not a safety plan.

Fever plus new stiffness is not a wait-until-Thursday observation. Send it the same hour and tell them to use emergency care. You are not diagnosing neuroleptic malignant syndrome. You are not sitting on a fever.

Where Emosapien fits

The score, the restlessness quote, and the return date have to sit in one record or the next visit starts from rumor. Emosapien keeps the medication lines and follow-up in the session record while you still ask the questions, interpret the number, and sign what is true.
