Jordan texts at 7:12 a.m.: “They put me on that new schizophrenia drug. I haven’t wanted breakfast and I feel weird in the bathroom.” You are not the prescriber. You still have a job. Name the product. Count what changed. Send a short observation to the person who can change the medication.

Every medication decision belongs to the prescriber. You observe, ask, document, and pass on.

What you ask after a switch

Keep the questions short enough that they fit before the work of the hour.

  • What is the name on the bottle or the injection card, and when did it start?
  • What did you notice in your stomach, urine, pulse, sleep, or energy this week?
  • Did you miss a capsule, or miss the injection window?
  • Any stiffness, restlessness, or a lump at a shot site?
  • Any new spending, gambling, or sexual drive that felt unlike you?
  • Any thought of being better off dead, or a plan?

You are not taking a vital sign. You are asking what the client noticed, in their words, so the prescriber has more than “seems to be doing well.”

If the same molecule is also used for depression in another client, do not collapse the two jobs. Schizophrenia coordination stays here. Adjunctive antipsychotic work for depression belongs on antipsychotics for depression.

What you write in the note

Date it. Name the product the client reports, generic and brand. Quote side effects in their words. Record missed doses or a missed injection window. Keep the therapy goal in behavioral terms. Put that observation on a psychiatric progress note template, not in a parallel spreadsheet you forget to send.

Do not write a milligram. Do not write “consider increasing.” Do not interpret labs you have not seen.

Cobenfy is the new schizophrenia drug that changed the mechanism. The FDA approval announcement (September 26, 2024) names xanomeline and trospium chloride for schizophrenia in adults, and calls it the first antipsychotic in this class that targets cholinergic receptors rather than dopamine receptors. The COBENFY label says efficacy is thought to come from agonist activity at M1 and M4 muscarinic receptors. Common reactions on that label (at least 5% and at least twice placebo) are nausea, dyspepsia, constipation, vomiting, hypertension, abdominal pain, diarrhea, tachycardia, dizziness, and reflux. Warnings include urinary retention, hepatic impairment, increased heart rate, reduced gastric movement, and angioedema. Those lines tell you what to ask. They do not tell you what to prescribe.

Free PDF: Patient and Psychiatry Note Skeletons

Two one-page fillable skeletons: a format-neutral patient note, and a psychiatry medication-management visit note.

  • Patient note: identifying line, reason for visit, observations, interventions, response, plan, signature
  • Format-neutral page. Not SOAP, DAP, BIRP, or GIRP
  • Medication visit: interval history, medications and adherence, side effects, mental status, risk
  • Assessment, medication changes, and follow-up on the same psychiatry page

What you pass to the prescriber

A useful pass-on is dated, names the medication, lists what the client noticed, flags missed doses, and states the therapy focus.

Same-day, not at the next med-check:

  • Cannot urinate, or a new slow stream the client is worried about
  • Swelling of face, lips, tongue, or throat
  • Yellow skin, dark urine, or unexplained itching
  • Fainting, or a pulse the client reports as newly fast
  • Fever with rigidity
  • New suicidal talk, plan, or a sudden drop in self-care
  • New gambling, spending, or sexual comments on aripiprazole

If GI effects are annoying and there is no red flag, send a dated note before the next med-check. Do not wait two more months to mention it.

When the new product is an injection

Uzedy, Erzofri, and Abilify Asimtufii are not new molecules. They are new delivery. The client may still call any of them a new schizophrenia drug. Track the shot date, the site, stiffness, restlessness, and whether the window was missed. The UZEDY label, the ERZOFRI label, and the ABILIFY ASIMTUFII label each carry the antipsychotic boxed warning on increased mortality in elderly patients with dementia-related psychosis, and none of those three products is approved for that use. If an older adult is on one of them for “agitation,” pass that frame to the prescriber.

Risk that does not wait

Voices getting quieter is not the only outcome that matters. Ask about hopelessness, plan, and means the week of a switch, then use the mental health risk assessment guide when the hour turns to suicide, violence, or inability to care for basic needs. A medication change does not replace that inquiry.

Keep the observation next to the note. Emosapien can carry the switch table, side-effect line, and prescriber handoff between sessions without moving them into a separate spreadsheet.
