Couples Therapy Progress Note Template

One partner is still listing unanswered evening texts. The other has gone still, voice flat, eyes on the floor. You named the cycle in the room. Now the official record has to hold both people without becoming a transcript, a treatment-plan rewrite, or a private process dump.

This couples therapy progress note template is for licensed therapists documenting an outpatient conjoint hour: who was present, identified-client or relationship frame, dual consent, each partner’s short report, the observed cycle, one intervention plus both responses, risk, and the next step.

What this couples therapy progress note template is and is not

This is the official two-client record for one conjoint hour. Another clinician covering next week should see the live goal, the cycle you observed, what you did, how each partner responded, and what happens next.

It is not a SOAP overlay and not a DAP reprint. When your setting requires SOAP, DAP, or BIRP headings, map these same two-client fields into that format. Use progress note templates and examples if you need those heading examples.

It is not a couples therapy treatment plan example. The plan holds shared goals across the course of care. This note records one hour toward a goal already on that plan.

It is not an emotionally focused couples therapy techniques tutorial. Cycle language in the chart names what you observed. It does not teach an enactment protocol.

It is not a family or household note, not a 90847 claim, not a group note, and not a private process note. Those jobs live elsewhere.

When this two-client chart fits

Use it for an outpatient conjoint hour when both partners are present, or for a documented one-partner hour that still sits inside an ongoing couples frame. Dual consent is already in place. You already decided whether one partner is the identified client or the relationship is the unit of care. The chart needs both voices without a transcript.

A useful hour for this skeleton looks like a communication cycle you can name in observable language: pursue-withdraw, attack-defend, or another pattern the couple recognizes. One intervention is enough. Both responses belong on the page even when they differ.

When to pause the couples note

Stop this skeleton when the hour is no longer conjoint couples work you can document as a shared official record.

APA Ethics Standard 10.02 requires you to clarify which individuals are clients, the psychologist’s role and relationship with each person, and the probable uses of the services or information when more than one person is in the room (Ethical Principles of Psychologists and Code of Conduct). Standard 4.02 requires you to discuss the limits of confidentiality. If those limits are not in place, do not write a shared couples chart as if they were.

A field is not done because it contains a sentence. “Processed communication” hides the cycle. “Both participated” hides who moved and who shut down.

Worked example

The worked example is a fictional conjoint hour with initials only that charts a communication cycle rather than an infidelity plot or an IPV vignette.

The note holds enough for continuity. It does not hold your working hypothesis about attachment history. That stays out of the shareable chart unless local policy requires it there for safety.

Privacy, dual-record, and safety

The HIPAA Privacy Rule at 45 CFR 164.501 already names joint counseling sessions in the psychotherapy-notes definition. Heightened protection applies only when a private record meets that definition and stays separate from the rest of the medical record. A folder titled “private” is not enough.

Sort after the hour:

• Continuity, intervention, both responses, risk, and plan stay in this official two-client chart.
• Cycle hypotheses, countertransference, and supervision prompts stay in a private process note if your practice uses that category.
• Graphic or extra detail with no care value stays out.

Use process notes vs progress notes when you sort official continuity from private process text. This template remains the shareable couples chart.

A covering clinician next week needs who was in the room, the live goal, the cycle you named, the intervention, both responses, risk, and the next step. They do not need your untested formulation.

Do not send this chart through ordinary consumer email or SMS. Do not treat the note as real-time monitoring. If IPV, coercion, or individual safety takes over the hour, leave conjoint documentation and follow the practice risk procedure.

APA Record Keeping Guidelines treat content, confidentiality, and retention as professional judgments, not as one universal couples template. Follow the controlling rule for your setting.
